The message is loud and clear – as the healthcare industry shifts to value-based healthcare and organizations are compensated for successful outcomes, providers must focus on effective management of patient populations. Less clear, however, is how to implement a successful population health strategy.

We polled 35 C-level healthcare executives at the Midas+ Annual Symposium to get their perspectives on population health management. The poll results were very insightful, but next week we will have the opportunity to continue and expand on the conversation during a live Google+ Hangout.

I am excited to be joined by four leaders who spend their days thinking about the next era of healthcare:

During our conversation, we’ll answer questions such as:

  • Why is population health management becoming more significant and necessary?
  • What type of health benefits can be achieved by leveraging clinical, behavioral and social insights?
  • What are some of the challenges providers face in implementing effective population health management? How can those challenges be overcome?
  • When will population health management be widely adopted throughout healthcare?
  • What does the future of healthcare look like with effective population health management?

But we also want to answer your questions. Use Twitter and hashtag #HealthITExperts to ask me, Deryk, Dan, Bernadette and Janice questions about population health management.
